The AR-1800-06 Transportal ACL Guide from Arthrex is designed to support surgeons performing anatomical femoral socket/tunnel creation during anterior cruciate ligament reconstruction using the anteromedial (“AM”) portal technique. With a 6 mm offset from the guide’s centerline, this instrument enables the surgeon to place the femoral tunnel more posterior and lower in the femoral footprint while maintaining an effective back-wall, helping to avoid blow-out and preserve bone integrity. Because the transportal approach offers more flexibility in tunnel orientation compared to traditional transtibial methods, the guide serves an important role in aligning drill paths with the native footprint of the ACL insertion.

In practical use, the instrument is placed against the femoral condyle through the anteromedial portal, aligned with arthroscopic landmarks, and a guide pin (or cannulated reamer) is inserted through the guide sleeve.
